Human Epididymal Protein 4 (HE4) is a glycoprotein secreted by the epididymal epithelium. It serves primarily as a tumor marker for clinical auxiliary diagnosis and monitoring. The normal reference range is generally 0–90 pmol/L, though slight variations may occur depending on the assay kit used. HE4 is a highly specific biomarker for epithelial ovarian cancer. Compared to the traditional CA125 marker, HE4 is less affected by benign lesions. It is frequently used alongside CA125 to calculate the ROMA (Risk of Ovarian Malignancy Algorithm) index, significantly improving early detection rates of ovarian cancer and detecting tumor recurrence 5–8 months earlier than conventional methods. Additionally, HE4 holds value in the differential diagnosis of endometrial carcinoma, cervical adenocarcinoma, and lung cancer.
