Prostate-Specific Antigen (PSA) is a family of alkaline enzymes secreted by prostate epithelial cells. It is present in prostate tissue and seminal fluid, with minimal levels in the serum of healthy individuals. PSA in serum exists primarily in three forms: ① Free PSA (fPSA), accounting for 10%–30% of total PSA (tPSA); ② PSA bound to α1-antichymotrypsin (PSA-ACT); and ③ PSA bound to α2-macroglobulin (PSA-α2M). The latter two are referred to as complexed PSA (cPSA). The primary physiological function of PSA is to prevent semen coagulation. It exhibits high tissue and organ specificity and is the biomarker of choice for diagnosing prostate cancer.
