Sulfonamides are folate synthesis inhibitors, consistently used due to their low cost and efficacy against pathogens causing urinary tract infections, trachoma, and other diseases. Their structure is similar to para-aminobenzoic acid (PABA), allowing them to competitively inhibit bacterial dihydropteroate synthase. This action prevents the synthesis of bacterial dihydrofolic acid, thereby inhibiting bacterial growth and reproduction. Sulfonamides are broad-spectrum antibacterial agents effective against both Gram-positive and Gram-negative bacteria, selectively inhibiting Streptococcus pyogenesStreptococcus pneumoniaeHaemophilus influenzaeEscherichia coliProteus mirabilisChlamydia trachomatisChlamydia lymphogranuloma venereum, as well as ActinomycesPneumocystis, and Nocardia.
Most systemically administered sulfonamides are readily absorbed orally and distribute widely into body tissues and fluids. They penetrate various extracellular fluids (pleural, peritoneal, synovial, aqueous humor), saliva, sweat, urine, and bile. They cross the blood-brain barrier, enter breast milk, and pass through the placenta.
