Spironolactone is a synthetic steroid compound, mainly used as a low‑efficacy diuretic. Its structure is similar to aldosterone, acting as a competitive aldosterone antagonist. Since it only affects the distal convoluted tubule and collecting duct, with no action on other segments of the renal tubule, its diuretic effect is relatively weak. Additionally, spironolactone also acts on aldosterone‑sensitive tissues outside the kidney.
On October 27, 2017, the World Health Organization’s International Agency for Research on Cancer (IARC) listed spironolactone in its preliminary compilation of Group 3 carcinogens.
