Lidocaine is a local anesthetic and antiarrhythmic agent, a derivative of cocaine without the hallucinogenic and addictive properties. Its hydrochloride salt is a white crystalline powder, very slightly soluble in water. Its toxicity is comparable to procaine, but it produces stronger and longer‑lasting local anesthesia with good surface penetration, and can be used both by injection and as a topical anesthetic. Lidocaine is an excellent local anesthetic, typically taking effect within 1‑3 minutes, with a duration of 1‑3 hours. It is also used to treat oral ulcers. As an antiarrhythmic, it is now less commonly used due to concerns about long‑term side effects. A small number of people are allergic to lidocaine. In 1963, it was introduced for treating arrhythmias and became the drug of choice for preventing and managing acute myocardial infarction‑related ventricular arrhythmias, including ventricular premature beats, ventricular tachycardia, and ventricular fibrillation.
