It inhibits protein synthesis by binding to the 50S subunit of the bacterial ribosome. It exhibits a significant post-antibiotic effect (PAE) and enhances the phagocytic activity of macrophages. Effective against Gram-positive bacteria, some Gram-negative bacteria, and Toxoplasma gondii, it achieves tissue concentrations 1.5-2 times higher than erythromycin, with biliary concentrations 7-10 times higher than serum levels, and can cross the blood-brain barrier.Primarily used to treat respiratory infections, otitis media, periodontitis, and toxoplasmosis caused by susceptible organisms. After oral administration, it is deacetylated into active metabolites. Compared to erythromycin, spiramycin achieves higher intracellular concentrations and has fewer side effects, though it may cause gastrointestinal reactions.
