Serum amyloid A (SAA) is an acute‑phase protein synthesized by the liver and associated with high‑density lipoprotein (HDL). In normal individuals, serum SAA levels are below 10 mg/L, but can rise 10‑ to 1000‑fold within 3–6 h after infection. Its degradation products can deposit in organs to form amyloid fibrils, which are associated with chronic inflammation and various diseases.
SAA is markedly elevated in bacterial, viral, and fungal infections, and is more sensitive than C‑reactive protein (CRP), rising earlier in viral infections and declining faster during recovery. Combined measurement of SAA, CRP, and white blood cells improves the identification of infection type and guides antibiotic use. SAA levels are also elevated in transplant rejection, malignancy, rheumatoid arthritis, and cardiovascular diseases, and are involved in pathological mechanisms such as atherosclerosis and resistance to immunotherapy.
